All of your medical records will be transferred to our new location automatically, at no cost to you. 

If you choose not to follow us and opt to find a new family physician instead, you may ask your new physician to fax us a chart transfer request with your signed consent and we can arrange to transfer your records accordingly. They may fax this request to Rideau-Rockcliffe Family Medicine to at 613-695-6464 after March 19th, 2018.
